2025-26 Vulnerable Sector Check Volunteer Letter Request
Enter the required information below to request a Volunteer Letter from the Tri-Centre Minor Hockey association.

The Ontario Hockey Federation (OHF) has centralized the screening process for all Team Officials and Team Personnel for the 2025-26 season. These individuals must adhere to the OHF Screening Policy. The OHF Screening Process involves the completion of the following two steps:

  1. Obtaining an approved Vulnerable Sector Check from your local police service. (PDF)
  2. Submitting the Vulnerable Sector Check results to the OHF Screening Submission Portal.

This is the first step in the two-step process.

Note: A Vulnerable Sector Check must be conducted by the police service that you are policed by. If you are unsure of who your police service is, click here and follow these instructions:

  1. Scroll down to the map in Step 1.
  2. Zoom into the area of your residence.
  3. Click on the approximate area of your residence.
  4. Note the police service that is displayed in the tool tip. This information is required later in the form below.
  5. Close the web page.

The Volunteer Letter is required before you apply for your Vulnerable Sector Check.

Personal Information

Enter your legal first and last name. The first and last name on the Volunteer Letter must match the legal name on the police check.

Volunteer Positions

Select the position(s) that you are volunteering for. You may be a volunteer for more than one team and hold different positions on those teams. The volunteer letter you receive will use the position with the highest level of risk.

Record Check

Criminal Record Checks including Vulnerable Sector Checks must be conducted by the police service that you are policed by. If you are unsure of who your police service is, review the note at the top of this form. It provides instructions on how to determine which police service you are policed by. Note: Ontario Provincial Police (OPP) Criminal Record Checks are only conducted online, Hanover Police Service Criminal Record Checks are only conducted in-person. Other police services offer the record check both online and in-person.
