Please complete this form to request a travel permit for any upcoming exhibition game played outside of your designated home centre. This form must be submitted at least 5 days in advance of the scheduled game to allow time for processing and approval.
Ensure all fields are filled out accurately, including full team names, game location, opponent details, and date/time. You will receive confirmation once your permit has been approved. If multiple games are planned on the same day or weekend, please submit one form per game.
